In the course of providing Services, Protonest may collect, create, receive, or maintain protected health information (“PHI”), to the extent Protonest is acting as a covered entity or business associate under the Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”), and applicable regulations.
This Notice of Privacy Practices (the “Notice”) describes how PHI may be used and disclosed by Protonest and explains your rights with respect to your PHI.
